To request changes, bring the following documents to the Department of Motor Vehicles (Narahenpita):

  • Copy of your National Identity Card
  • Sworn petition confirming your residence (if the address is different)

  • Leasing company’s certified copy (if applicable)

Once there, you’ll need to:

  1. Fill out the relevant form provided at the DMV.
  2. Submit documents at the counter and collect the receipt.
